On May 23rd, all individuals wanting to play any sport at Cleveland High School will have the opportunity to get a free physical for the upcoming 2012-2013 school year. Physicals will be available from 1-4 p.m. in the Jones Wrestling Center. This will be the only date that the high school will offer free physicals this year. Anyone that does not attend the physicals on this date will be responsible for getting a physical from their primary physician on their own.

Forms are available through your team’s coach. Contact your coach to pick up these forms if you plan to get your physical on your own. A parent signature and medical history must be filled out before you receive your free physical.
