| If it is necessary for your child to take medication
at school, please be sure to fill out the proper
medication form.You may pick one up from the teacher or
in the office, or you may print out one from the link
provided.We want to be sure that all medications are
given properly. No medication will be dispensed to
students by school personnel without this form being on
file. Thanks for your cooperation in this matter. This applies to prescription as well as non-prescription medicines. The forms can be printed from here. |
